How we process your data…

When you complete a referral form, we will ask you why you are contacting the Careers Champions service, how you heard about us, and what sort of advice you are looking for. We will also ask you for information to help us arrange a careers conversation at the most suitable time for you. The only personal details we will be collecting is your name, preferred means of contact and details of your current job. We would encourage you to provide the usual Equality & Diversity information to help us monitor access to the Careers Champions service. This is entirely optional and will be deleted once we have captured the data and within 72 hours of our receiving your referral form.

Immediately after the careers conversation, your Careers Champion will capture the outcome of the conversation and offer you the opportunity to complete a customer satisfaction survey. This is optional, entirely anonymous, and has just 4 questions! Your careers conversation will remain entirely between you and your Careers Champion; what happens next and who you talk to is entirely up to you.

Three months after your careers conversation your Careers Champion will contact you to see how you are getting on, what you have done since the careers conversation, and what job you are doing now. Once we have received this information, and no later than 4 months after receiving your original referral, your information will be deleted. However, the Workforce team at Norfolk & Waveney Health and Care Partnership (NWHCP) will retain the anonymised information to enable evaluation of the service and inform operational improvements.

Any information you share with NWHCP for the purposes of the Careers Champions service will be stored securely and electronically within a restricted area of the network only accessible by the designated members of the Workforce team supporting this service. Your information will be treated as confidential and will only be used within the scope of the Careers Champions service. Your personal information will not be shared with any third parties.

By completing and submitting a referral form to the Careers Champions service, you are giving your consent for your details to be held securely by NWHCP. If you no longer wish to access the service, you may withdraw your referral at any time by advising NWHCP at nwcareers.champions@nhs.net. On receipt of your request, any information held about you will be deleted and your referral will not be progressed.
